This is a public ticket. Everybody will be able to see its contents. Do not include usernames, passwords or any other sensitive information.

Potentially you could join multiple rating questions, using the suppress question feature on the second and subsequent questions. You may need to play with the CSS to get it looking like a single question.
You can then make some of these rating questions mandatory and some not.
