You can use the "BF My Quiz Plus" module to display all quizzes that are available on your site.

What you are after should be possible, but you'd need to make some custom modifications to the module.

Perhaps you could insert your image etc in the description field within the quiz category, then adjust the query in /modules/mod_bfmyquiz_plus/helper.php to include the description field. Maybe something like:

$query->from($db->quoteName('#__categories'));
$query->select('id, title, description');
$query->where('extension = "com_bfquiz_plus"');

You'd then need to put this data in the lists variable, maybe something like:
$lists[$i]->description= htmlspecialchars( $row->description );

You could then adjust the output in /modules/mod_bfmyquiz_plus/tmpl/default.php to use this description field to display as desired.

I hope this helps.

When you have entered the score next to each option, make sure that you don't have any leading or trailing spaces, and this can cause this error. Also you should only be using numbers, unless you are using the ABCD answer matrix scoring type, in which case instead of using numbers you would use the letters A, B, C or D.
